All of the documents below must be downloaded in order to register.
Directions on how to complete the forms and their due dates are listed at the top of each document. Please take note that some forms are to be filled out electronically and emailed back to

and other forms are to be printed, completed and mailed or brought to check-in.
To download the documents:
1. Click on the icon next to the title of the document.
2. A pop-up box will appear asking you to open or save the document. Save the document in a place where you are able to access the form in the future.
3. You may be prompted after the form has been saved to Open, Open Folder or Close the document. Click the Open button if you would like to open the document up. Click the Open Folder button if you would like to open the folder where the document is saved. Click the Cancel button to close the pop-up screen.
4. For the Word documents, enter the information in each of the gray shaded areas. Please note the specific directions on each document. After completing the form, please EMAIL the forms as attachments to

Program Fee: $925.00
- Checks should be made payable to the Aquatennial Ambassador Organization.
- If you are unable to attend Candidate Orientation, please send the entry fee
to: Jennifer Brentano, Aquatennial Ambassador Organization, PO Box 293, Chaska, MN 55318.
- The full entry fee MUST BE RECEIVED by the AAO Office by May 1, 2012.
- See Entry Requirements Doc item#17 for more details.
- If a candidate finds it necessary to withdraw from the competition, her entry fee is nonrefundable.
